This European Resuscitation Council (ERC) Guidelines 2025 Adult Advanced Life Support (ALS) includes the advanced interventions that can be used in addition to basic life support (BLS) and automated external defibrillator (AED) by health care professionals. These ALS treatments are helpful when started quickly and early during cardiac arrest. ALS includes the prevention and treatment of both in-hospital cardiac arrest (IHCA) and out-of-hospital cardiac arrest (OHCA), the ALS algorithm, manual defibrillation, airway management during cardiopulmonary resuscitation (CPR), drugs and their delivery, and the treatment of peri-arrest arrhythmias. These ERC Guidelines 2025 are based on the International Liaison Committee on Resuscitation (ILCOR) Consensus on Science and Treatment Recommendations (CoSTR) for ALS.1 For these ERC Guidelines, the ILCOR recommendations were supplemented by focused literature reviews undertaken by the ERC ALS Writing Group for those topics not reviewed by ILCOR. When required, the guidelines were informed by the expert consensus of the writing group membership. For the first time we have had a patient public representative (GC) on the ALS Writing Group.
